Saline Rather Then Silicone Gel Breast Implants

It is unusual to find an article on silicone gel breast implants that I completely agree with. I have found such an article. The article is titled silicone gel breast implants are back? So what?”

 

The opinions in the article about silicone gel breast implants are based upon extensive experience. I find the author’s logic faultless. It’s best to read his words.

Saline Rather Then Silicone Gel Breast Implants 

 

“There’s an opinion that’s asserted by breast implant manufacturers — and echoed by many physicians — that silicone gel-filled implants are better than saline-filled implants. In my opinion, “it ain’t necessarily so.”

Since the FDA moratorium on silicone gel-filled implants began in 1992, I, like many plastic surgeons in the United States, had been limited to using saline implants for primary breast augmentation surgery. Between 1992 and 2008, I placed Mentor smooth round saline implants in more than 3,500 patients.

In November 2006, the FDA approved silicone gel implants. Soon after, the San Diego Union-Tribune reported “that plastic surgeons said they would quickly become preferred among the nearly 300,000 women in this country who have breast implants each year.” Mentor called the decision a “historic moment,” while Allergan, the other silicone gel implant manufacturer, said it “created new options for women.”The main selling point was that silicone gel implants feel more natural.

In the two-year period from November 2006 through October 2008, only 4 of my 709 patients requested silicone gel-filled implants. They have not become “preferred” over silicone implants.”

“The saline implants, which are empty, can be folded and placed through a small incision. My incisions are usually about 3 cm, whereas a pre-filled silicone-gel implant requires a 5-7 cm incision on average. Also, it’s much easier, and requires less dissection, to put an empty implant through an incision than to try to manipulate — without breaking — a gel-filled implant.

In my experience, rippling is a very rare occurrence, even nine months or more after surgery when the skin is fully stretched, the muscle fully relaxed and the breasts completely healed into their final shape. Rippling might occur if a patient has had a significant weight loss with a corresponding loss of body fat, or if moderately droopy breasts get significantly droopier and skin becomes very thin after childbirth.

If a woman were dissatisfied with the feel or look of the rippling, she would be a good candidate for an exchange to a silicone gel-filled implant, but only at this point.

When a saline implant deflates, the breast returns to its original size, and the deflation is very obvious. When a silicone gel implant ruptures, the cohesive gel relatively maintains its shape, and there is usually no physical sign of this change. So, to evaluate these implants for rupture, implant companies recommend that patients have an MRI three years after surgery and every two years thereafter. These MRIs are not covered by insurance.

To top it off, silicone gel implants cost about ,000 more per pair than saline implants.

Saline vs. silicone? If a patient requests silicone gel implants, no problem. But if a patient asks my opinion, I recommend saline, without hesitation.”

The one time I recommend silicone gel breast implants is when a patient is absolutely flat. In a situation where the chest is ‘flat like a boy’ silicon gel breast implants may be the preferred choice.

“However, there is an advantage to using fat because most of the breast is made of this in the first place. It feels more natural because it is natural.

Despite these advantages, caution is advised. We do not know the long term effects of large volume fat injection into the breast. It is probably safe, but studies are looking at this. Smaller volumes of fat injection are frequently used as a supplemental procedure in breast reconstruction. It is a fantastic way to fine tune the shape of a reconstructed breast.

One concern is that injected fat does not always survive. When it dies, it can harden from calcium crystals forming in the expired clumps of fat. When you are using the larger volumes of fat required for cosmetic breast enlargement, this creates two potential problems. First, you may feel these calcified areas as lumps. Second, these areas can be seen on mammograms, which may interfere with breast cancer surveillance. These issues are not generally a problem when the fat is injected in tiny amounts. Small particles of fat tend to survive well.

Beyond safety are some practical issues. It takes many hours to perform this procedure since the fat must be injected in minute quantities at a time. It can take as many as six or eight hours. This means that the procedure will cost much more than a one to two hour breast augmentation with implants. Additionally, since all of the fat may not survive transfer, multiple procedures may be necessary to get enough fat to create the right size. Thin women may not even have enough extra fat to achieve the size they want.”

The key point that is made is that breast augmentation using grafted fat is not fully tested yet. It does seem to be effective. It also has some significant drawbacks using present technology. It cannot be done with every patient. Some patients may not have enough fat to allow for breast augmentation using grafted fat.

Clinical Trials Cohesive Gel Breast Implant

Allyson Quesada recently wrote an article on the cohesive gel breast implant. The FDA for routine use in the United States has not approved the cohesive gel breast implant. They have been used for 20 years in many areas around the world. The cohesive gel breast implant are undergoing clinical trial in the United States.

“The term ‘cohesive gel’ refers to the gel’s thickness – how much it bonds together. The thicker the gel, the more cohesive it is. In the case of cohesive gel implants, the gel is a solid mass of gel. Its cohesiveness is such that there is no liquid element whatsoever. With a cohesive device, you cannot squeeze it and displace its filler in the same manner that you can saline devices or traditional silicone devices.

Traditional breast implants are available in two design styles: round and anatomical. The new cohesive gel device is only available in an anatomical shape. The reason why is because its solid form of gel is immune to the pull of gravity; if the implant were spherically shaped it would look abnormal on a woman’s chest because it would never change shape regardless of how the woman positioned her body.

As mentioned above, the new cohesive device does not contain liquid. A non-liquid breast implant is the ideal solution for implant rupture and leakage. For years, the traditional silicone gel implant device has been the subject of extreme controversy over safety concerns relating to the interaction of its silicone gel and the human body. The new cohesive gel breast implant eliminates all leakage concerns simply because it does not contain any liquid whatsoever and therefore it cannot leak its filler into the human body.

The solid nature of the gel and the fact that the implant is always produced in an anatomical shape leads to one potential problem: the implant device will always maintain its shape and form regardless of the pull of gravity. Thus, a woman standing on her head will have the same aesthetic breast appearance as a woman standing in the upright position. Is this a benefit or a drawback? That’s for you to decide.

The human body has a defense mechanism when a foreign object enters the body. It will form a layer of scar tissue around the object and isolate it from coming into contact with the body. When this occurs with a breast implant device, the condition is referred to as capsular contracture. The condition occurs infrequently with both saline and traditional silicone gel devices. The new cohesive gel device appears to be impervious to capsular contracture, at least according to early data collected throughout the world.

There are two negative traits associated with the cohesive gel device. The first is its potential for surgical scarring. Because the device is a solid mass of gel it requires larger than normal incisions in order for it to be inserted into the breast pocket. The second negative trait is if the device rotates it will require corrective surgery in order to restore the implant to its proper position. Of course, all anatomically shaped implants will require corrective surgery should the implant rotate within the breast pocket (not just cohesive implants).”

The cohesive gel breast implant is often referred to as the Gummy Bear implant. This is because its feel is very much like the candy of the same name.

 

The cohesive gel breast implant when used for breast augmentation may have a firmer less natural feel than other implants.

“This may be a somewhat controversial statement, but in my opinion there currently is really only one acceptable position for breast implants in an elective cosmetic surgery patient: behind the pectoralis major muscle (sub-pectoral augmentation). There are a number of very compelling reasons to place implants behind the pec major, and the most compelling one of all is the fact that radiologists report that the mammographic imaging of breasts for the purpose of breast cancer screening tends to be more easily accomplished when breast implants are sub-pectoral (compared to pre-pectoral, also referred to as the ‘sub-mammary’ position). An American woman’s current lifetime risk of breast cancer is approximately 1 in 8 to 1 in 9, so the issue of breast cancer screening must be taken very seriously. Mammography is by no means a perfect screening study, but it is the standard of care at this point in time. The most sensitive and specific test for breast cancer is a contrast-enhanced MRI scan, and breast implants do not impair breast tissue visualization by MRI.

The next very compelling reason to select sub-pectoral placement is a cosmetic one. Implants placed on top of the pec major tend to stand out in the upper pole of the breast, creating a rounded, convex and distinctly unnatural-appearing breast profile. With implants in a sub-pectoral position, the upper pole of the implant is flattened somewhat by the muscle, helping to create a smooth transition from the area in the upper chest where the breast begins, and a gradual slope towards the nipple that is not excessively rounded or convex. In some patients with a fuller breast volume preoperatively one may get an acceptable appearance with pre-pectoral placement – initially. The problem is that as breasts age, the fatty tissue atrophies and breast tissue thins out, and the area where this is most obvious is in the upper pole and cleavage area. So a pre-pectoral implant that was initially well-concealed may, after a few years, become painfully obvious (including visible implant folds and ripples) in the upper pole.”

 

Dr. Paul Zweibel from Denver answers:

“Under the muscle is best for:

  • thin women
  • women with little breast tissue to cover the implant
  • women with little or no drooping of the breast
  • softer, more natural draping of the implant
  • concealing implant ripples
  • less chance of developing capsule contracture

Over the muscle is best for:

  • faster recovery
  • women with enough natural breast tissue to cover the implant
  • women with drooping who want larger breasts without a breast lift”

The plastic surgeons gave 4 reasons that it is best beneath chest muscle with breast implants. Those reasons are: there is less rippling, there is less chance of contracture, the breast looks more natural, and mammograms more effective.

 

There were two reasons they felt it was not best beneath chest muscle with breast implants. Those reasons are: when breasts sagging was present and when rapid recovery was the key issue.

After looking at the statistics I’m amazed. I expected to answer the question ’is breast augmentation more likely in certain parts of the country’  with a yes. I expected there to be a difference yet not as dramatic a difference. I would sure like to know if the average age of the patients that had breast augmentation is different in various sections of the country? There are many different ways these statistics can be looked at. I wonder if breast augmentation is less accepted in certain parts of the country? What are the cultural differences that caused this striking difference in statistics?

If infection occurs around the breast implant the breast implant needs to be removed. It is then usually replaced after six months time. The incidence of infection around a breast implant is less than 1%.

 

The other short-term breast augmentation risk is bleeding. This is commonly called a hematoma. This occurs in approximately 4% of women who have breast augmentation. The treatment of a hematoma is to remove the blood collection and replace the implant.

 

The long-term breast augmentation risks are capsular contracture, asymmetry, and deflation.
